Dear Parents, Guardians & Caregivers,
My name is Ivanice Kaplan. I will be your nurse during this school year. It is a pleasure to work with you and your family. My job is to support you and your child’s health and learning, and I hope we can all succeed during this school year. I look forward to meeting you in person when possible or needed. If your child has any new health conditions, such as asthma, allergies, diabetes, and seizures, please bring the Emergency Action Plan and the medication to the main office or health office when possible. Please click below:
We encourage parents and guardians to administer all medications at home. However, it may not be possible. Any medication that needs to be administered by the School Nurse must be prescribed by a Physician.
Medication administration
The Medication consent form must be completed and signed by the parent/guardian and the physician. The medication must be brought to the Health Office in the original pharmacy container labeled with the student’s name and details for administration.
Students from outside of the district need to have updated Medical Documentation records. Please email the records to the health office or upload them on Genesis documents.
It is important to share your child’s annual physical with updated immunization records.
We strive for 100% immunization compliance every year. You are welcome to bring the record, email it or download it in Genesis Documents.
I like to send emails to parents if a child gets hurt at school. Please share if the best way to communicate with you is by phone or email.
